HOW TO MAKE YOUR  DEDICATION

  The Tree of Light is the perfect way to make a dedication.

The form we use is suitable for all kinds of dedications and you will find some guidance on the form about how your dedication could be shown.

Forms are available at the Ludlow Visitor Information Centre,  at Ludlow Library, and from members of The Rotary Club of Ludlow. Please tell your friends how to obtain them, or print off extra copies for them.

All you need to do is complete the form (block capitals please) with your dedication(s) and your name and address and send it with your cheque to one of the addresses shown.

You can put more than one dedication on a page, and use as many pages as you like, but please remember to donate at least £5 with each dedication you make. 

If you have an email address we can use it to acknowledge your donation, to save postage.   If you are a tax payer, we ask you to sign the Gift Aid Donation section, (this is of course voluntary) which makes your donation worth so much more to the charities we are supporting!

For £5, a light symbolises your dedication, and all proceeds will be divided equally between:

• Ludlow and District Riding for the Disabled;
• MacMillan Cancer Support;
• Midlands Air Ambulance;   and
The Town Mayor’s Charity  " Working together".
